These services offer individualized community-based support for people, 16 years of age or over, suffering from a serious mental illness. CASE MANAGEMENT Service provided is client directed and aimed at improved quality of life through community integration. Case Management provides ongoing support to people affected with serious mental illness. This may include: Community connections, housing, financial planning, income budgeting, family relationships, and education about mental illness. COURT SUPPORT SERVICES Specialized Court Support Case Managers provide assistance to people with mental illness who come in conflict with the law. These additional supports may include: Court diversion plans, pre-charge diversion, release from detention facilities, and support and consultation to staff working in the criminal justice system. CONCURRENT DISORDERS CASE MANAGEMENT A Concurrent Disorders Case Manager provides assessment and linkages to treatment and rehabilitation services for people with mental illness who also have an addiction to drugs and alcohol.
